JOB SUMMARY: This job is for the care of Kaiser Permanente (KP) members in the Skilled Nursing Facility (SNF) setting. The provider will manage the care of the member, collaborate with other KP colleagues on the SNF rounding team, and interact with the SNF facility staff.
