    Registered Nurse Case Manager

    The Registered Nurse Case Manager's primary purpose is to provide nursing care to acute and chronically ill clients of all ages in their homes utilizing nursing principles.

    Job Description

    • Assume full responsibility for a client from admission to discharge as a Case Manager.
    • Identify differences in skilled nursing as defined by third party payers and professional nursing practices.
    • Ensure coordination of visits after hours and on weekends and for overflow visits during the week.
    • Develop and modify the plan of care in coordination with the physician and client.
    • Coordinate ancillary services (i.e. therapies, home care aides).
    • Complete all required documentation according to nursing standards of practice, agency policies, and reimbursement requirements by third party payers.
    • Assist in the implementation of quality assurance and safety procedures.
    • Interpret home care policies and procedures to personnel, clients, and family members as necessary.
    • Review home care policies, procedures, manuals, and job descriptions on a regular basis.
    • Maintain an adequate liaison with families and clients.
    • Make written and oral reports concerning observations and recommendations to the Supervisor concerning home care aide functioning.
    • Responsible for appropriate delegation of assigned tasks to home care aides, companions, and licensed nurses.
    • Other functions as deemed necessary and appropriate or as may be directed by your supervisor.
    • Must have the ability to work well with others as a team.
