Job Description

• Provide primary care to adult patients, with an emphasis on prevention and long-term condition management
• Collaborate with care teams to address medical, behavioral, and social needs
• Build trusted relationships through continuity of care
• Maintain quality documentation with EMR support
• Contribute to a patient-first model focused on outcomes, not volume

Requirements

• Board-certified or board-eligible in Family Medicine or Internal Medicine
• Active or in-process Alabama medical license
• Strong interpersonal and clinical decision-making skills
• Commitment to community-focused, patient-first care
