An established outpatient primary care clinic on the South Side of Chicago is seeking a Board-Certified or Board-Eligible Internal Medicine or Family Medicine Physician to care for a predominantly geriatric, Medicare-focused patient population. Job Description

• Provide comprehensive outpatient primary care to older adults
• Manage chronic conditions such as diabetes, hypertension, CHF, COPD, and arthritis
• Emphasize preventive care, care coordination, and patient education
• Collaborate closely with nursing staff, medical assistants, and care coordinators
• Practice within a value-based care model focused on outcomes, not volume

Requirements

• MD or DO
• Board-Certified or Board-Eligible in Internal Medicine or Family Medicine
• Active Illinois medical license or ability to obtain
• Interest in outpatient, geriatric-focused primary care
