Spanish-speaking Nurse Practitioner

An outpatient primary care organization is seeking a Spanish-speaking Nurse Practitioner to join a value-based clinic in Brooklyn, New York. This clinic focuses on providing preventative and chronic care management for Medicare patients through a collaborative team model and smaller patient panels. The care model prioritizes patient outcomes, preventative medicine, and stronger provider-patient relationships.

Job Description

• Schedule: Monday to Friday
• Hours: 8:00 AM to 5:00 PM
• Setting: Outpatient primary care clinic
• Patient population: Primarily geriatric / Medicare
• Visit length: 30 minutes
• Patient panel capped around 450
• EMR: Epic

Clinical Support

Each provider receives:

• Dedicated medical assistant
• Dedicated scribe
• Shared RN support
• Collaborative physician and APP care teams

This structure allows providers to focus on quality patient care rather than high-volume visits.

Responsibilities

• Provide comprehensive primary care services
• Diagnose and treat acute and chronic conditions
• Conduct preventative health visits
• Educate patients on disease management and lifestyle improvements
• Document encounters in Epic EMR

Requirements

• Family Nurse Practitioner (FNP) or Adult-Gerontology NP
• Board Certified
• Active New York NP license
• Active DEA license
• Fluent or professional Spanish proficiency required

Experience with Medicare or geriatric patient populations preferred. New graduates may be considered.
