Primary Care Provider

Annashae is seeking a Primary Care provider for a mental health facility near Carbondale, IL. This is a full-time position with hours Monday-Friday from 6:30 AM to 3:00 PM (with flexibility), no weekends, no call, and no holidays. We provide malpractice insurance and can credential quickly.

Job Description

• Provide treatment to a caseload of patients on the pavilion
• Evaluate assigned cases at intake
• Sign certificate of admission and testify in court
• Order and interpret lab tests
• Conduct initial physical exams (H&P) and make diagnoses of patients
• Develop and implement treatment plans
• Follow up exams and document changes in progress notes
• Prescribe and direct administration of medicines
• Monitor effectiveness of medication

Requirements

• Current unrestricted Illinois License
• Board Certified/Eligible in Internal or Family Medicine
• Current Illinois Controlled Substance certificate
• Current unrestricted DEA certificate
• Mantoux skin test prior to starting work or documentation of negative Mantoux within the last 90 days
• Clearance of required fingerprints and drug screen
