Advanced Practice Provider - Primary Care

Join our team as an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) in Wolfeboro, NH, located 50 miles from Manchester, NH. This full-time, permanent position offers the opportunity to provide high-quality, compassionate care in a collaborative primary care setting.

Job Description

• Conduct physical exams, take patient histories, and order diagnostic tests
• Develop and implement care plans in collaboration with physicians
• Educate patients on treatment plans, medications, and health maintenance
• Document care in the EMR accurately and thoroughly
• Coordinate referrals and follow-up care
• Maintain patient confidentiality and follow HIPAA guidelines
• Participate in staff meetings and continuing education

Requirements

• Graduate of an accredited NP or PA program
• Active state license and DEA registration
• Minimum of 3 years experience preferred

Skills & Abilities

• Strong communication and critical thinking skills
• Comfortable with EMR systems (Epic preferred)
• Ability to work independently and within a team

Physical Requirements

• Light lifting (up to 25 lbs), standing, typing
• Able to manage mental and emotional demands of clinical care
