Geriatric Nurse Practitioner or Physician Assistant

Hartford HealthCare is seeking a full-time, experienced Nurse Practitioner or Physician Assistant for our REACH (Resource for Elderly And Care at Home) Program serving our geriatric patients in the greater Willimantic area. This position offers a weekday schedule with home-based care in the greater Willimantic area.

Job Description

• Provide comprehensive care to geriatric patients in their homes
• Collaborate with a multidisciplinary team to ensure optimal patient outcomes

Requirements

• Unrestricted State of Connecticut Medical License (or eligible)
• APRN (Family, Adult/Geri Nurse Practitioner) or PA training
• At least two years of post-graduate experience as a Nurse Practitioner (APRN) or Physician Assistant (PA)
