Physical Therapist

Physical Therapist needed for home health setting surrounding the Colchester, VT area! This is a 20-week assignment. The facility uses OASIS, and experience is strongly preferred.

Job Description

• Must have home health experience
• Average caseload is 18-22 clients, equivalent to 5 visits per day
• Mileage is usually limited to a specific area of the county (30-mile radius)
• Shift: Monday-Friday, 8 AM-5 PM
• Patient population includes adults (ages 30-100) with conditions such as cardiac issues, pulmonary conditions, diabetes, wound care, post-surgery recovery, orthopedics, neurological impairment, palliative care, dementia, CVA, SCI, COPD, Parkinson's disease, ostomy care, catheters, and MVA recovery

Requirements

• Licenses/Certifications: CPR/BLS (Basic Life Support), State License - Physical Therapist - Vermont, NPI Number
• Physical abilities: Must be able to lift from 50-100 lbs
