Job Description

• Provides direct patient care activities to home care or home health patients according to physician orders in the home environment including assessment, evaluation, treatment planning, implementation, and instruction/supervision of physical therapy practice.
• Instructs and assists patients to perform various physical activities and in the use of assistive and supportive devices such as crutches, canes, and prostheses.
• Instructs patients and families in treatment procedures.
• Re-evaluates patients and adapts treatment programs accordingly to achieve maximum benefit.
• Confers with physicians and other practitioners to obtain additional information, suggest revisions in treatment plans, and integrate physical therapy treatment with other aspects of patients' care.
• Evaluates, fits, and adjusts prosthetic and orthotic devices and recommends modifications to Orthotist.

Requirements

• Bachelor's Degree in Physical Therapy (PT)
• Valid Driver's License
• Physical Therapist License

Schedule

• Full time
• Day shift
• 8 a.m. - 4:30 p.m.
• Weekend/Holiday rotation
