    Join our dynamic team as an RN at Evelyn's House, a 36-hour night position from 7 p.m. to 7:30 a.m.

    Job Description

    • Evaluate the client and furnish services requiring substantial and specialized skill, appropriate preventive and rehabilitative nursing procedures, and instructions to assist the client in learning appropriate self-care techniques.
    • When assigned as case manager, the staff nurse is responsible for coordinating all aspects of care related to that patient.
    • Assess patient preferences and barriers to involvement in care, including their values, emotional, spiritual, cultural, and population-specific needs.
    • Develop, implement, and document individual plans of care with defined goals in collaboration with other members of the interprofessional team and patient, family, or caregiver in accordance with the established guidelines and standards of nursing care. Proactively plan and ensure communication of the plan of care across the continuum of care.
    • Promote respect, equity, and empathy in interactions with diverse and vulnerable populations through care delivery (e.g., support for emotional, spiritual, and cultural preferences of patient, family, and/or caregivers). Practice collaborative problem solving, service recovery, and advocacy for patient family-centered continuity of care. Implement care by integrating data from the interprofessional team and critical thinking in a safe and timely manner.
    • Evaluate changes in patient's condition, inform and collaborate with family and/or caregivers, and communicate with the interprofessional team as changes occur in the plan of care. Update the plan of care in EMR. Evaluate current nursing care to ensure evidence-based practice and quality patient outcomes.
    • BJC has determined this is a safety-sensitive position. The ability to work in a constant state of alertness and in a safe manner is an essential function of this job.

    Requirements

    • Education: Nursing Diploma/Associate's in Nursing
    • Experience: No experience required
    • Licenses & Certifications: Valid Driver's License, RN

    Preferred Requirements

    • Education: Bachelor's Degree in Nursing/Home Health
    • Experience: 2-5 years
