• Provide comprehensive primary care exclusively for seniors (65+ Medicare/Medicare Advantage)

Requirements

• Licensed APRN in Florida and graduate of an accredited nursing program
• 1-3 years of primary senior care experience preferred
• Confident managing chronic conditions such as diabetes, COPD, CHF, and hypertension
• Proficient in EMR documentation with strong organizational skills
• A natural communicator with genuine compassion and strong follow-through
• Passionate about senior-focused, value-based care and patient advocacy
