Job Description

• Provide comprehensive outpatient care to adult and senior patients
• Diagnose and manage complex medical conditions
• Maintain accurate documentation and coordinate necessary referrals
• Collaborate with an interdisciplinary care team
• Participate in quality improvement and clinical initiatives

Requirements

• MD or DO; active, unrestricted medical license
• Board Certified in Family Medicine, Internal Medicine, or Geriatric Medicine
• Minimum 2 years of experience in value-based care or high acuity geriatric populations
• Commitment to patient-centered, outcomes-driven care
• Participation in TB screening

Preferred Qualifications

• Training in Family Medicine, Internal Medicine, Med Peds, or Geriatrics
• Experience in senior care or value-based care settings
• Proficiency with EHR systems
• Strong communication and team collaboration skills
• Experience supervising NPs/PAs
