Staff Pharmacist
The staff Pharmacist is responsible for providing medication therapy and management services in collaboration with other providers to assure that drug therapy is safe, effective, efficient, and cost-effective.
Pharmacy services provided include interdisciplinary patient care rounds, medication reconciliation, medication therapy monitoring, medication orders management, patient education, drug information, drug dosing, and various other activities.
Performs both clinical and distributive tasks necessary to ensure that patients' medication therapy is safe, effective, timely, and efficient.
May be asked to provide training and education to PharmD students through practical and didactic experiences.
Acts independently within the scope of their license and areas of expertise with the help of general policies, procedures, and practice guidelines with support from the Director of Pharmacy.
Ensures that patient care is maintained through compliance with the department's policies, rules, and regulations promulgated by the Board of Pharmacy and other applicable regulatory bodies.
Acts as Antimicrobial Stewardship Pharmacist as assigned.
Requirements
- Graduate from an accredited college of Pharmacy
- Currently licensed as a pharmacist in the State of Nebraska or ability to obtain it within 3 months of hire
Functional Requirements
- Able to communicate in the English language and make assessments by speaking, hearing, seeing, feeling, reading, and writing
- Works primarily in a hospital setting, including clinical/patient care areas and office or meeting areas
- Considerable walking, sitting, or standing for long periods of time
- Mental effort required to assess, plan, and make recommended decisions in a complex environment
- Potential to be exposed to blood/body fluids, including infectious diseases
- Must be able to maneuver throughout halls, stairways, and patient rooms in response to emergencies
- Interpretation of environmental input requires visual and auditory skills
- In the event there is a need to evacuate the building, lifting will be required to transport patients to safety
- Significant amount of keyboarding and writing is required
- Wearing of personal protective equipment (PPE) such as gloves, masks, gowns, and goggles may be required
- Fine motor skills
- Long periods of intense concentration
Physical Requirements

Lifting: May be required to lift 32 lbs. from floor to waist height on an occasional basis; may be required to lift 25 lbs. from floor to above shoulder on an occasional basis.

Push/Pull: May be required to push/pull 30 lbs. with load on floor in squatting position on an occasional basis; may be required to push/pull 10 lbs. distance >110 feet on an occasional basis.

Standing/Walking/Sitting: Must be able to stand 5-8 hours; must be able to walk 3-5 hours; must be able to sit 1-3 hours.

Lower Extremities: Must be able to squat/bend/kneel on an occasional basis; may be required to use a step stool (1 step) on an occasional basis.

Upper Extremities: Must be able to grip/grasp desk items/paper on a frequent basis; must be able to grip/grasp/pinch meds bottles/med boat on a frequent basis; must be able to perform extended reach overhead; must be able to perform extended reach at waist level; must be able to bend (at 90 degrees) while reaching.

Equipment: Must be able to use a computer and perform keyboarding on an occasional basis.

Environmental Factors: Must be able to work a 10-hour shift; may be required to work in a noisy environment on an occasional basis.

Interprets the appropriate life span needs of patients, provides appropriate population-based care according to the department skills checklist for identified populations and age groups identified below:
- Neonate: Birth to 27 days of age
- Pediatric: Greater than 27 days of age to 13 years of age
- Adult: Greater than 13 years of age to 65 years of age
- Geriatric: >65 years of age

Must demonstrate knowledge of the principles of growth and development over the life span of patients/families and possess the ability to assess data reflective of the patient's life span.

Work Practice
- Order review and entry.
- Dispenses and compounds medication based on provider orders.
- Consults with physicians regarding medications for pediatric, adolescent, adult, or geriatric patients as appropriate.
- Checks medications filled by Pharmacy Tech/Pharmacist for accuracy.
- Completes admission and discharge medication reconciliations as needed.
- Provides educational instruction to patients, nurses, providers, and public as needed.
- Assists in maintenance of inventory and purchasing.
- Participation on committees.
- Completes training to compound Sterile, Hazardous, and Non-Sterile medications.
- Follows department and hospital policies and procedures.
- Knowledgeable of pharmaceutical waste.
- Attends Code situations (Cardio-Pulmonary arrests) and Trauma I situations.
- Proficient in all functions of pharmacy computer system (Paragon Pharmacy).
- Proficient in all pharmacy programs (i.e., Pyxis, Pyxis Logistics, PharmacyKeeper, Sentri7, Control Check, DoseMeRx, QS/1).
- Assists with pharmacy students or job shadows.
- Keeps accurate records relative to State and Federal Regulations.
- Responsible for function of department in absence of Director.

- Demonstrates appropriate competencies in the department with initial orientation competency due within 90 days of hire and annual competencies to be completed by September 30th.
- Maintains competency through completing education assigned on CCH learning management systems as well as mandatory in-person education.
- Complies with Attendance Management Policy in regards to attendance and timeliness, understanding the expectation of maintaining an absence percentage of 3% or less.
- Accepts other duties within scope of practice at the direction of the person-in-charge.
