Job Description

• Provide clinical and operational support for the ambulatory clinics in the Specialty Pharmacy service line.
• Responsible for some inpatient job responsibilities such as inpatient order set verification.
• Create and maintain clinical pharmacy pathways for initiation and monitoring of specialty medication therapy for self-administered medications, including step therapy requirements, patient education, and monitoring of applicable laboratory, pharmacokinetic, and pharmacogenomics results.
• Treat patients that require medication therapy for conditions such as Hepatitis C, Cancer, Cystic Fibrosis, and HIV.
• Knowledgeable in benefits investigation, financial support, clinical monitoring, education, and dispensing a variety of sub-specialty services.
• Available in the evenings and weekends as needed.

Requirements

• Earned a Bachelor's degree from an accredited pharmacy program; Doctorate degree preferred.
• Currently hold an active and valid Indiana Pharmacist license.
• Four (4) to five (5) years of experience in a specialty, ambulatory, hospital-based, or a closed-door long-term care pharmacy.
