Family Medicine or Internal Medicine Physician

Founded by our Central WA Family Medicine Residency program (CWFM) in 1993, CHCW has grown into an integrated community health center to include full spectrum primary care, pediatric, senior residential care, behavioral and addiction care, and dental serving the Yakima and Kittitas Counties at multiple sites.
Our Senior and Residential Care program (SR Care) needs a Family Medicine or Internal Medicine Physician with a passion to care for the elderly/senior population, or Geriatrician to provide primary care services to SR Care clients at participating LTC facilities and serve as the individual SNF's Medical Director.
Job Description
- Enjoy a flexible, full-time, 4-5 day workweek with admin time, no after-hours call, no weekends
- Integration and management of care to include inpatient admitting, clinical, and admin services
- Shine as the Medical Director to our contracted SNFs
- Advocacy for patients and providers with administration round tables for input with strategic planning
- Share your passion with residents and medical students in a teaching-friendly culture
- Expected volume of seeing 14 patients assigned per day, typically at 1 facility per day
- CHCW has been a trusted partner in the health and wellbeing of our communities for over 30 years
- Use of modern technology: GERIMED
- Collaborative support from the SR Care Program Director, residents, physicians, ARNPs, medical assistants, and program assistants
Compensation and Benefits
- Base salary: $237,500 - $247,500. Additional financial add-ons include a quality/engagement incentive, $10k fellowship bonus (if applicable), $4k per SNF oversight (7 contracted SNF facilities shared), and production bonuses

Requirements
- MD/DO degree or IMG equivalent
- Completion of an accredited US FM or IM Residency
- Preference for geriatric medicine certification, or FM/IM with added qualifications in geriatrics, and/or AMDA certification or training
- WA License or the ability to obtain a WA license
- DEA certificate
- BE/BC required
- Great peer references and in good standing
- Approved for credentialing with Medicare, Medicaid, and health plans
