Job Description

• Monday - Friday, 7am - 4pm or 8am - 5pm shifts with rotating call coverage
• Primary caseload of 45 direct care patients with potential for 25 additional patients
• Long-term care facility with dedicated rehabilitation and dementia units
• Works alongside attending physician with PT/OT support available
• Manages wound care, IV medications, and therapy coordination
• Handles admissions, discharges, and emergency transfers
• Provides care for diverse patient population from TBI to complex chronic cases
• Weekly rotating call schedule with weekday and weekend coverage
