Join our team as a Registered Nurse (RN) Case Manager in a PRN/Hourly position. This role is essential in the care of hospitalized patients, working from 8 AM to 4:30 PM, seven days a week.

Job Description

• Case Management plays an important role in the care of hospitalized patients. Our team of RN Case Managers, Social Workers, Utilization Review staff, and Discharge Planners assure that payer requirements are met and develop an individualized plan of care for our patients.
• The Case Management staff coordinates with the interdisciplinary team and community resources to maximize the patient's opportunity to succeed.
• You'll make an impact by utilizing your specialized plan-of-care intervention and serving as a patient-care innovator. You will shape exceptional patient journeys every day and leverage your skills and our cutting-edge technology to directly impact patient wellbeing.

Requirements

• Current state RN license
• Associate's or bachelor's degree from an accredited nursing school
• BLS Certification
• 3 years experience in acute care as a Professional Nurse or RN experience with significant acute care hospital Case Management experience
• Previous case management experience preferred
• Case management certification, CP preferred
