Family Medicine Physician and Faculty Member

Join our South Bend, IN team! We are looking for a passionate full-time Family Practice Physician with faculty duties to join an outpatient clinic in South Bend, IN.

Job Description

• Spend two clinical days per week managing patient care.
• Develop effective care plans and ensure continuity of care through proper scheduling and follow-ups.
• Provide whole-person care by coordinating with social workers, nutritionists, and mental health professionals.
• Maintain thorough and accurate documentation and conduct periodic health exams.
• Educate patients about their health.
• Oversee infection control and residents in hospital settings.
• Engage in Total Quality Management (TQM) activities, attend meetings, and teach students and residents.
• Assist with rotations, counsel residents on academic matters, participate in program reviews, and contribute to the recruitment and selection of new candidates.

Requirements

• Medical degree from an accredited medical school.
• Board certification or eligibility in Family Medicine with 1-3 years of experience.
• Active medical license in the state of Indiana.
• DEA registration for prescribing controlled substances.
• Core faculty member for a minimum of 1 year.
