Join our team at Essentia Health in Duluth, Minnesota, as a Locum Tenens Geriatric / SNF Provider. This position offers a full-time schedule with no nights, weekends, or call, allowing for a balanced work-life experience.

Job Description

• Monday to Friday schedule
• 8:00 AM to 5:00 PM
• Full-time required (this position is not part-time or flexible days)
• Daily visits and follow-up care in Skilled Nursing Facilities and Long-Term Care/Nursing Homes
• Manage acute and chronic conditions in geriatric patients
• Provide continuity of care across multiple facilities throughout the week
• Coordinate care with nursing staff, facility leadership, and Essentia Health teams
• Perform assessments, medication management, care planning, and documentation
• Focus on high-quality, patient-centered care for elderly populations

Requirements

• Board Certified or Board Eligible in:
  • Geriatric Medicine
  • Internal Medicine
  • Family Medicine
• Required experience in at least one of the following:
  • Skilled Nursing Facility (SNF)
  • Nursing Home
  • Long-Term Care (LTC)
• Must be comfortable working independently in SNF/LTC settings
• Ability to manage complex geriatric patients

Credentialing

• Assignment begins upon completion of credentialing
• Candidate will be screened by agency first, then interviewed by facility
