Job Description
- The Family Nurse Practitioner provides generalized health care services; is under the supervision and reports directly to the supervising licensed physician.
- The Nurse Practitioner assumes principal responsibility, functioning with approved protocols for health maintenance and clinical management of acute and chronically ill patients.
- Identifies the health status of patients by taking medical histories, performing examinations, and initiating and evaluating screening and diagnostic procedures.
- Initiates appropriate therapeutic intervention based upon assessment and evaluation of the health status of the individual.
- Determines which cases to refer to the licensed physician for further diagnosis and treatment; serves as clinical consultant to nursing and paramedical personnel; and coordinates work activities with families, community agencies, and other members of the health team.
- The Family Nurse Practitioner is a member of the patient care team and is involved in the coordination of patient care.
- Participates in annual trainings, such as: CME and the Patient Centered Medical Home (PCMH).
- Performs other duties as assigned.
Requirements
- Mature judgment, tact, discretion, and confidentiality are mandatory.
- Requires a professional, pleasant, and effective representation of the corporation.
- Bilingual (Spanish/English) preferred.
- Must possess a dependable automobile with insurance and a valid California driver's license.
- Must submit to and pass a urinalysis test, fitness for duty, criminal background check, and credentialing clearance as a precondition for employment.
- Must be able to provide proof of COVID-19 vaccination and booster, recent TB test, MMR, TDAP, and Hepatitis B vaccines.
- BLS/CPR certification required.
- Must be a certified Licensed Nurse Practitioner. An RN with at least a Master's Degree in Nursing and advanced education in primary care.
