Job Description
- Provide comprehensive in-home care and care coordination for medically complex patients.
- Conduct initial assessments, follow-ups, and hospital transition visits.
- Perform full vitals and head-to-toe physical exams.
- Manage medications and chronic care plans.
- Provide patient education on chronic disease management.
- Collaborate in team rounds discussing high-risk patients.
- Document visits in Athena with real-time care team visibility.
Daily Patient Load
- 4-5 visits per day
- New patient visits: 90 minutes
- Follow-ups: 30-60 minutes
Schedule & Flexibility
- Hybrid Schedule
- 3-4 days: In-home patient visits
- 1-2 days: Remote admin, charting, or telehealth

Compensation & Benefits
- Base Salary: $120,000 - $145,000 (DOE)

Requirements
- Nurse Practitioner or Physician Assistant license
- 2+ years of Advanced Practice experience
- Experience managing care plans or chronic disease patients preferred
