    APP (NP / PA) - In-Home Chronic Care

    Job Description

    • Provide comprehensive in-home care and care coordination for medically complex patients.
    • Conduct initial assessments, follow-ups, and hospital transition visits.
    • Perform full vitals and head-to-toe physical exams.
    • Manage medications and chronic care plans.
    • Provide patient education on chronic disease management.
    • Collaborate in team rounds discussing high-risk patients.
    • Document visits in Athena with real-time care team visibility.

    Daily Patient Load

    • 4-5 visits per day
    • New patient visits: 90 minutes
    • Follow-ups: 30-60 minutes

    Schedule & Flexibility

    • Hybrid Schedule
      • 3-4 days: In-home patient visits
      • 1-2 days: Remote admin, charting, or telehealth

    No nights

    No weekends

    No on-call

    Compensation & Benefits

    • Base Salary: $120,000 - $145,000 (DOE)

    Requirements

    • Nurse Practitioner or Physician Assistant license
    • 2+ years of Advanced Practice experience
    • Experience managing care plans or chronic disease patients preferred
