    Palliative Care Physician Value-Based Care Hybrid

    An innovative, national healthcare organization is seeking a Board-Certified Palliative Care Physician to join a forward-thinking, value-based care model focused on improving quality of life for patients with chronic and advanced illnesses.

    This position offers a unique opportunity to practice within an integrated, multidisciplinary team that delivers in-home and virtual palliative care designed to enhance patient outcomes, reduce hospitalizations, and improve overall patient and family experience.

    Job Description

    • Full-time, Monday to Friday schedule
    • No nights, weekends, or call
    • Hybrid model: 60% remote (telehealth), 40% in-person home visits
    • Clinical position with a focus on patient care and quality outcomes
    • Variable daily volume (approximately 3 to 7 patients/day via telehealth)

    Key Responsibilities

    • Identify and manage patients appropriate for palliative care services, building and maintaining a dedicated patient panel.
    • Provide symptom management for pain, depression, anxiety, insomnia, and other conditions impacting quality of life.
    • Conduct goals-of-care and family meetings; document wishes in advance directive and POLST forms.
    • Support hospice transitions where clinically appropriate.
    • Collaborate with interdisciplinary teams, including nurses, social workers, chaplains, and specialists.
    • Foster strong relationships with local primary care providers, nephrologists, and community partners.
    • Participate in performance reviews and quality initiatives to meet both clinical and financial KPIs.
    • Educate and mentor care teams on palliative care principles and best practices.

    Requirements

    • Doctorate Degree (MD or DO) from an accredited medical school
    • Board Certification in Hospice and Palliative Medicine
    • Current, unrestricted medical license (multi-state licensure preferred or willingness to obtain)
    • Strong background in symptom management, advance care planning, and patient-centered communication
    • Experience with Medicare Advantage, managed Medicaid, and population health models preferred
    • Familiarity with integrating acute, behavioral, and social care needs
    • Reliable transportation required
