Job Description

• Directs, establishes, and plans the overall policies and goals for the Social Services Department
• Responsible for assessments, care planning, and helping residents adjust
• Supervision of Department Staff
• Discharge planning at the start of admission
• Occasional community outreach
• Familiar with a variety of concepts, practices, and procedures
• A wide degree of creativity and latitude is expected
• Relies on extensive experience and judgment to plan and accomplish goals

Requirements

• 2+ years of management experience in long-term care settings
• LSW - MSW
• Knowledge of applicable state and federal guidelines
• Exceptional interpersonal and leadership skills
