Nurse Practitioner / Physician Assistant - Skilled Nursing Facility Care
We are seeking a Nurse Practitioner or Physician Assistant to deliver clinical care in skilled nursing and post-acute care facilities across Mesa, Gilbert, and Chandler on a weekend-only basis. This independent contractor position is ideal for providers who value flexibility, autonomy, and the opportunity to make a meaningful impact in long-term care settings. You'll play a key role in enhancing patient outcomes, supporting continuity of care, and reducing avoidable hospitalizations.
Job Description
- Conduct thorough weekend patient rounds, including clinical assessments and care reviews
- Manage acute conditions and chronic disease exacerbations
- Order, interpret, and act on labs, imaging, and other diagnostics
- Initiate or adjust medications as clinically appropriate
- Coordinate care with facility staff, physicians, and specialists
- Educate patients, families, and caregivers on treatment plans and expectations
- Perform in-scope procedures based on credentials and facility needs
- Document all patient encounters accurately and promptly in the EMR
- Adhere to SNF-specific protocols, regulatory standards, and best practices
- Support transition planning and referral coordination when needed
Requirements
- Active and unrestricted Arizona NP or PA license
- Graduate of a U.S.-based, accredited Nurse Practitioner or Physician Assistant program (Note: Programs in Puerto Rico are not accepted)
- 1-2 years of experience in post-acute or SNF care strongly preferred
- Familiarity with EMR systems and SNF care workflows
- Strong communication, clinical judgment, and care coordination skills
