    Job Description

    • Interpret orders and transcribe or verify them accurately and efficiently in the pharmacy computer system.
    • Accurately compound and dispense pharmaceuticals.
    • Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose.
    • Provide accurate and timely medication information to healthcare staff and patients.
    • Effectively and proactively interact with medical staff regarding drug therapy issues.
    • Document all clinical activities and interventions accurately and completely.
    • Sustain the hospital formulary, minimizing non-formulary procurements, utilizing therapeutic interchanges and other approved formulary protocols and/or guardrails.
    • Supervise and assist pharmacy technicians and other support staff effectively.
    • Train and participate with Disaster Team drills and real-event response as assigned.
    • Prioritize work and lead others to comply with facility medication turn-around time policy and procedure and identify areas for improved efficiency.
    • Actively participate in staff meetings and attend staff development in pharmaceutical care to identify learning and performance needs.
    • Support in the training and mentorship of new staff members as assigned.
    • Must serve as preceptor for pharmacy students and/or residents.
    • Support and participate in the goals of the organization's quality improvement, risk management, and patient safety program to include but not limited to National Patient Safety Goals, Core Measures, and HCAHPS medication-related initiatives.
    • Maintain professional competency sufficient to meet current and future needs of the facility.
    • Maintain appropriate controlled substance records.
    • Comply with all regulatory standards.
    • Make appropriate evidence-based, patient-centered medication recommendations.
    • Participate in the management of medical emergencies as appropriate, including but not limited to Code Blue.
    • Provide clinical consultation and clarification to practitioners. Suggest appropriate, cost-effective therapeutic alternatives to medical staff as needed.
    • Work with the Pharmacy Director or Operations Manager to identify and develop processes to meet or exceed all regulatory standards and participate in quality and regulatory reviews and audits.

    Requirements

    • B.S. or Pharm D. from an ACPE-accredited School of Pharmacy.
    • Clinical pharmacy experience in a hospital setting.
      • Completion of an accredited PGY-1 Pharmacy residency or fellowship.

    OR

    • 2+ years' clinical pharmacy experience with no history of residency or fellowship.

    • Active Pharmacist license in good standing with the appropriate State Board of Pharmacy.

    • Other applicable state requirements - i.e., sterile compounding, etc.

    • BLS & ALS/ACLS certification (BLS required within 90 days of hire; ALS/ACLS required for attending Code Blue).

    Preferred Requirements

    • Doctor of Pharmacy from an ACPE-accredited college of pharmacy.
