A large, mission-driven Federally Qualified Health Center (FQHC) is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join its outpatient primary care team. Job Description

• 100% outpatient primary care
• Adult and pediatric patient population
• Team-based care model with physicians, APPs, nursing, behavioral health, and care coordinators
• Mission-driven organization with strong leadership and clinical support
• Predictable weekday schedule (no nights or weekends)

Responsibilities

• Provide high-quality outpatient primary care services to patients across the lifespan
• Diagnose, treat, and manage acute and chronic conditions
• Perform routine physical exams, preventive care, and health screenings
• Develop and implement treatment plans and patient education
• Document care in the electronic medical record (EMR)
• Collaborate with physicians and interdisciplinary care teams
• Participate in quality improvement initiatives and population health efforts

Requirements

• Licensed Nurse Practitioner or Physician Assistant
• Board certified or board eligible
• Active or eligible state license
• Primary care or family medicine experience preferred
• Commitment to community-based and underserved care

Benefits

• Competitive salary range: $120,000 - $140,000
