A beautiful facility near Baker City, OR is seeking a full-time Physical Therapist to join their team!

Job Description

• Pay: $40.00 - $57.00 hourly
• $10,000 retention bonus + relocation assistance on a case-by-case basis!
• Full-time, permanent, direct-hire
• Schedule: Day shift, 5x8 or 4x10
• In-home patient visits are required for the Physical Therapist.
• Evaluate mobility, strength, balance, and functional abilities to determine care needs.
• The Physical Therapist will develop and implement individualized treatment plans in collaboration with the patient, family, and care team.
• Provide skilled, goal-oriented physical therapy with ongoing assessment and plan adjustments.
• Document care per regulatory guidelines and coordinate with interdisciplinary teams.

Requirements

• Bachelor's degree from an accredited Physical Therapy program
• Valid PT license, or eligibility to obtain
• BLS Certification, or eligibility to obtain
