    Physical Therapist

    Job Description

    • Manage the complete therapeutic care of an assigned group of patients, coordinating with physicians and the patient care team to ensure the best outcomes.
    • Focus on individualized care plans aimed at achieving patient goals, enhancing engagement, and ensuring a positive patient experience.
    • Support patients through the continuum of care, delivering high-quality, patient-centered services.

    Key Responsibilities

    • Patient Evaluation and Treatment: Provide evaluations and skilled therapeutic interventions in accordance with the scope of practice and professional ethics.
    • Therapeutic Assessments: Assess and evaluate the rehabilitative and functional status of patients, contributing to the development of their individualized care plans.
    • Care Implementation: Deliver purposeful therapy interventions to produce positive changes in patients' conditions and functions, aligned with the physician's plan of care.
    • Patient and Family Education: Instruct patients, families, and caregivers on the use of therapeutic exercises, appliances, and/or prosthetic devices for use outside of therapy sessions.
    • Caseload Management: Prioritize patient caseloads, determine staffing needs, and coordinate daily assignments and patient handoffs to ensure effective care delivery.

    Requirements

    • Valid Physical Therapy license.
    • Bachelor's Degree in Physical Therapy or related field.
    • Basic Cardiac Life Support (BCLS) certification required within 30 days of employment start date.
    • Previous experience in a physical therapy setting is preferred but not required.
