    Case Management Assistant

    Care Management is looking for a smiling face with customer service experience to work in the hospital setting. If you are outgoing and upbeat, this position is for you! You must enjoy working with people and have a caring attitude!

    Job Description

    • Actively participate and assist with the patient and caregivers bedside assessment and coordination of post-acute resources in line with the established plan of care.
    • Ensure patients receive post-acute care from quality and valued providers while taking the lead in the coordination of follow-up appointments.
    • Assist in the readiness of the patient for discharge and participate in the communication to the patient and caregiver on the post-acute plan.
    • Assist with preference based on quality, lead the referral process, and provide resources to patients as directed by the Care Manager.
    • Participate in the delivery and act as a resource in compliance and regulatory requirements as part of the Conditions of Participation.

    Requirements

    • High School Diploma or equivalent required.
    • Associates or Medical Assistant degree preferred.
    • 1 year of previous clerical or secretarial experience required.
    • Hospital or post-acute experience preferred.
    • Basic knowledge of Microsoft Office.
    • Excellent communication and customer service skills.
    • Ability to multitask responsibilities.
    • Calm, professional demeanor.
