Requirements

• Provider must have a clean file and be available for 7 or more shifts per month.
• CV must have all dates in MM/YY to MM/YY format.
• Work history must be in sequential order with the most recent on top.
• Any gaps over 30 days must be explained on the CV.
• All licenses, boards, and certifications with expiration dates must be listed on CV.
• Anyone with malpractice must provide an NPDB.
• Anyone with legal issues must provide supporting documents.
