    Job Description

    The Nurse Practitioner (APRN) will provide high-quality primary care to adult and senior patients by conducting physical exams, ordering and interpreting diagnostic tests, and developing individualized treatment plans in collaboration with a supervising physician. This is a safety-sensitive position involving direct patient care and medication administration.

    • Provide comprehensive care under physician supervision using sound medical judgment and ethical practice.
    • Manage patients with acute and chronic illnesses, focusing on the older adult population.
    • Maintain accountability for patient care and outcomes, consulting specialists when necessary.
    • Coordinate with hospitalists when patients are hospitalized.
    • Ensure compliance with regulatory standards (HIPAA, CLIA, state/federal regulations).
    • Educate patients on prevention, risk reduction, and chronic disease management.
    • Maintain accurate EMR documentation within 48 hours of each patient visit.
    • Uphold patient confidentiality and adhere to all internal policies and procedures.
    • Perform additional duties as assigned.

    Requirements

    • Required: Bilingual (English/Spanish) proficiency
    • Graduate of an accredited nursing program
    • Certified and licensed Nurse Practitioner in the state of practice
    • Minimum 1 year of nursing experience; 1-3 years as a practicing NP preferred
    • Experience in primary care or geriatrics highly valued
    • Proficiency in EMR systems and standard office software (Google Suite, Microsoft Office)
    • ACLS/BLS certification required
    • Strong communication, problem-solving, and organizational skills

    Physical Requirements

    • Ability to stand, walk, and assist patients for extended periods
    • Lift/carry up to 15 lbs and operate medical equipment as needed
    • Visual and hearing acuity required for patient care
    • Must have reliable transportation
