Join our team at the Center for Chronic Care Management as a LCSW Care Manager. This part-time position focuses on providing support in the management of anxiety and depression for patients dealing with chronic disease, as well as support and education for patients who wish to quit tobacco addiction.

Job Description

• Provide support in the management of anxiety and depression for patients with chronic diseases
• Offer support and education for patients seeking to quit tobacco addiction

Requirements

• Licensed Clinical Social Worker (LCSW) able to practice in the State of Connecticut
• Minimum of 5 years of experience in acute care, home care, and outpatient settings
• Willingness to obtain Certification as Tobacco Treatment Specialist within 1 year of hire

Preferred Qualifications

• Certification as a Tobacco Treatment Specialist (CTTS) is preferred
• Care management or care coordination experience is desired
