Outpatient Physical Therapist

Join our team in the Physical Rehab department, where we provide exceptional outpatient services. This position offers a full-time schedule with a focus on patient-centered care.

Job Description

• Administer Physical Therapy services under medical prescription in accordance with Hospital regulations, State of Connecticut Practice Act, and quality standards.
• Develop a plan of care for each patient with the patient's input and agreement, addressing safety, educational, psychological, and developmental needs.
• Interpret and assess data about the patient, setting and revising goals throughout the plan of care while explaining treatment goals, plans, techniques, and outcomes to patients and/or families.
• Recommend various therapeutic procedures and modalities.
• Comply with professional and departmental documentation standards.
• Address a home exercise program with each patient.
• Maintain an adequate caseload, adhering to departmental productivity standards.
• Attend continuing education seminars as required by licensure and present educational programs to Rehab staff or other medical professionals.
• Communicate regularly with the Rehabilitation Supervisor, other team members, and referring physicians for patient care.
• Perform all other job-related duties as assigned.

Requirements

• Graduate of an accredited School of Physical Therapy.
• Licensed or eligible for licensure to practice in the State of Connecticut.

Preferred Skills

• Experience with electronic medical charting.
• 3-5 years of clinical experience.
• Flexibility with scheduling.
