    Job Description

    • Clinician member of the MultiCare Medical Partners multidisciplinary team delivering population-based care.
    • Provide comprehensive primary care services focusing on prevention, chronic disease management, and health promotion for diverse patient populations.
    • Collaborate with physicians, RNs, pharmacists, social workers, behavioral health specialists, and other professionals to optimize the health of MultiCare communities.
    • Conduct comprehensive assessments, diagnose conditions, develop plans, and prescribe medications for patients across the lifespan.
    • Diagnose and treat acute and chronic disease using evidence-based guidelines.
    • Provide preventive care, health education, and counseling to individuals and families.
    • Ensure timely and accurate documentation, inclusive of direct patient encounters and asynchronous care.
    • Participate in the coverage for clinic colleagues per departmental guidance.
    • Participate in after-hours call coverage per departmental guidance.
    • Engage in multidisciplinary meetings, care reviews, and care planning.
    • Coordinate care with all members of the delivery team.
    • Contribute to the development and improvement of population-based care.
    • Utilize data and risk stratification tools to prioritize patients at greatest risk for outreach.
    • Engage in quality improvement focused on health outcomes, cost-effectiveness, and cost reduction.
    • Support transitions of care and reduced readmissions and ED use.
    • Demonstrate the MultiCare values of compassion, stewardship, kindness, joy, collaboration, and respect.
    • Adhere to MultiCare policies and processes.

    Requirements

    • Maintains active Washington State Nurse Practitioner or Physician Assistant license.
    • DEA and state prescriptive authority.
    • Current BLS for Healthcare Providers certification by the American Heart Association.
