Job Description

• Part-time post-acute care position in a skilled nursing/inpatient setting
• 2 days per week schedule: Tuesdays and Thursdays
• Primary care focused role with flexibility and strong work-life balance
• Manage both acute and chronic conditions for post-acute patients
• Develop individualized care plans and collaborate closely with the interdisciplinary care team
• Work in partnership with an on-site physician for clinical support
• Well-structured onboarding and strong back-office infrastructure
