Job Description

• Leadership role overseeing post-acute care clinicians in the Albany area
• Conduct clinical case reviews and support care coordination initiatives
• Collaborate with pharmacists and care teams to improve patient outcomes
• Focus on quality, readmission reduction, and individualized patient care
• $25,000 sign-on bonus

Requirements

• Board Certified in Internal Medicine, Family Medicine, or related field
• Prior experience as Medical Director in a skilled nursing facility preferred
• Active New York license and DEA
• Experience in post-acute, hospice, home health, or hospital medicine a plus
