Job Description

• Manage patient admissions and discharges
• Provide medication management and treatment oversight
• Evaluate and respond to changes in patient condition
• Conduct wound care evaluations and coordinate referrals when needed
• Maintain accurate and timely documentation of patient care
• Collaborate with physicians and care teams to support patient outcomes

Requirements

• Active Nurse Practitioner license in New York
• Minimum 2 years of experience in a hospital or subacute rehabilitation setting
• Strong clinical assessment and decision-making skills
• Excellent communication and organizational abilities
