    Job Description

    • Conduct comprehensive physical exams and patient assessments
    • Diagnose and manage acute, episodic, and chronic medical conditions
    • Prescribe medications and other evidence-based treatments
    • Develop and implement individualized patient management plans
    • Educate patients and families on wellness, prevention, and early disease detection
    • Order, perform, and interpret diagnostic tests and laboratory results
    • Collaborate with physicians and multidisciplinary healthcare professionals to ensure coordinated, high-quality care
    • Maintain accurate and timely patient records in compliance with HIPAA
    • Participate in quality improvement initiatives and professional development opportunities

    Requirements

    • Current and unrestricted Nurse Practitioner license in the state of New York
    • 2-3 years of experience in a primary care setting
    • Medicaid credentials required
    • Spanish language proficiency a plus
    • Comfortable with electronic health record (EHR) systems
