Job Description

• Conduct thorough patient evaluations, focusing on wound assessment and management
• Develop and implement individualized wound care treatment plans
• Perform wound care procedures, including cleaning, dressing changes, and monitoring healing progress
• Educate patients and families on wound care management, prevention, and self-care strategies
• Collaborate with physicians, nurses, and interdisciplinary teams to ensure coordinated patient care
• Maintain accurate and timely documentation in electronic medical records
• Monitor patient progress and adjust treatment plans to support optimal outcomes

Requirements

• Current and unrestricted Nurse Practitioner license in Illinois
• Wound Care certification preferred (company support available for certification within first 9 months)
• Minimum 2 years of clinical experience in wound care or a related specialty
• Strong clinical skills in wound assessment and treatment planning
• Excellent communication and patient education abilities
• Ability to work independently and collaboratively within a healthcare team
• Energetic, self-motivated, and highly interested in wound care
