Registered Nurse (RN) Home Health

We are seeking a Registered Nurse (RN) with home health experience to join our team serving the Clearwater area. In this role, you will provide skilled nursing services to patients in their homes, including some individuals residing in assisted living facilities. The ideal candidate is comfortable working independently, managing a caseload, and delivering high-quality patient care through clinical treatment, disease management, and patient education.

Job Description

• Provide wound care and administer IV medications as ordered
• Perform disease management and monitor patient conditions
• Oversee medication management and ensure adherence to treatment plans
• Educate patients and families on treatment plans, medications, and home care
• Maintain accurate and timely documentation of patient care
• Coordinate care with physicians, caregivers, and healthcare team members
• Travel to patient homes, including assisted living facilities, within the service area

Requirements

• Active Registered Nurse (RN) license in Florida
• Home health experience required
• Strong clinical, organizational, and patient education skills
• Ability to work independently and manage patient visits effectively
• Excellent communication skills with patients, families, and care teams

Benefits

• Compensation: $70,000 - $75,000 per year (DOE)
• 12 days of PTO in the first year
• Sick Time Off (STO)
• Mileage reimbursement at $0.50 per mile
• Monday - Friday schedule
• On-call rotation every 4 - 5 weeks (phone only)
