We are seeking a dedicated Registered Nurse (RN) with home health experience to join our Waco team. This role involves conducting start-of-care, recertification, and resumption-of-care visits, providing wound care, educating patients and families, and accurately documenting all care in the EMR.

Job Description

• Conduct start-of-care, recertification, and resumption-of-care assessments
• Complete OASIS documentation for all patient visits
• Provide wound care and perform supervisory visits as needed
• Educate patients and families on care plans and treatment
• Accurately document all patient interactions using Homecare Homebase EMR
• Deliver compassionate, culturally sensitive, patient-centered care

Requirements

• Active Texas RN license
• Minimum 1 year of home health experience
• Familiarity with Homecare Homebase EMR system
• Strong communication, clinical assessment, and organizational skills
• Ability to work independently and manage time effectively
