Job Description

• The Home Health Nurse provides skilled nursing care to patients in their homes, focusing on promoting health, preventing disease, and helping patients manage chronic conditions.
• This role involves coordinating care with physicians, therapists, and other healthcare professionals to ensure a high standard of care and patient safety.

Requirements

• Minimum of 1 year of current work experience providing in home health.
• Pennsylvania State Healthcare Provider license or willing to obtain one.

Certifications Needed

• This position may require one or more of these certifications: BLS.
