    Job Description:

    • Responsible for implementing standards of care for physical therapy services
    • Complete initial assessments and routine visits
    • Communicate all patient-related information back to the office and document accordingly

    Requirements:

    • Currently licensed as a Physical Therapist in the State of VA
    • Preferred two years of experience as an occupational therapist; Home Health experience preferred
    • Good verbal and written communication and organization skills
    • Strong computer skills; familiarity with Allscripts/NetSmart preferred

    Essential Job Functions:

    • Provide physical therapy services according to a written physician's plan of care
      • Assess and evaluate therapeutic/rehabilitative/functional status and participate in the development of the total plan of care
      • Evaluate the home environment
      • Assess for muscle strength, mobility, gait, and ROM-potential for rehabilitation
      • Direct physical therapy treatment
      • Instruct patients and families/caregivers in the use and care of therapeutic appliances
      • Determine priority needs for physical therapy
      • Report to the physician patient's reaction to treatment or changes in condition
    • Initiate physical therapy program and instruct other personnel and/or family/caregiver members
    • May train patients in the use of prosthetic devices
    • Supervise physical therapy assistant, home health aides/personal care aides
    • Identify patient and family/caregiver needs for other home health services and refer as necessary
    • Prepare and submit clinical and progress summaries based on the attainment of goals
    • Participate in discharge planning for the patient
    • Provide physical therapy consultation to families/caregivers when indicated
    • Provide in-service education for nursing personnel as needed
    • Participate in peer consultation process
    • Perform other duties as assigned

    Education:

    • Master's (Preferred)

    License/Certification:

    • Physical Therapy License (Required for State of VA)
    • CPR Certification (Required)
