Provider will see ages 2 and up for Primary Care, acute episodic care, physicals, and wellness visits. Hours are Monday 8:00am-4:00pm, Tuesday 10:00am-6:00pm, Wednesday 7:00am-3:00pm, Thursday 9:00am-5:00pm, and Friday 6:00am-2:00pm, all with a 30-minute lunch break.
