Nurse Practitioner (NP) / Physician Assistant (PA) - Primary Care
We are a small, close-knit primary care practice specializing in family medicine and committed to providing high-quality, patient-centered care. We are seeking a skilled and dedicated NP or PA who shares our passion for promoting wellness, managing chronic conditions, and delivering comprehensive outpatient services to individuals of all ages.
Job Description
- Conduct thorough patient assessments, including medical history, physical exams, and diagnostic testing.
- Diagnose and treat common acute illnesses, injuries, and chronic medical conditions.
- Develop and manage individualized treatment plans, including medication and lifestyle recommendations.
- Provide preventive care, health education, and counseling to support overall wellness.
- Manage follow-up visits, monitor progress, and adjust care plans as needed.
- Partner closely with the primary care physician and healthcare team to deliver coordinated, holistic care.
- Refer patients to specialists when necessary and consult on complex cases.
- Participate in team meetings, case reviews, and quality improvement initiatives.
- Maintain accurate, up-to-date electronic medical records (EMRs).
- Adhere to all regulatory and documentation standards.
- Build trust-based relationships with patients and families.
- Provide education on disease prevention, chronic disease management, and healthy lifestyle practices.
- Stay updated on the latest evidence-based care through continuing education.
Requirements
- Current NJ license as a Nurse Practitioner (Family NP) or Physician Assistant.
- Master's degree in Nursing (NP) or graduation from an accredited PA program.
- Current BLS and ACLS certifications (if applicable).
- Minimum 1 year of primary care or family medicine experience (new graduates not preferred).
- Strong diagnostic, clinical, and EMR skills.
- Excellent communication and interpersonal abilities.
- Experience in mental health and aesthetics is a plus.
Role Details
- Setting: Outpatient primary care
- Schedule: Monday-Friday, 9:00 AM - 5:00 PM
- On-Call/Weekends/Nights: None
- Patient Volume: 25 patients/day per NP/PA (staff ratio 12:1)
- Patient Age Range: Pre-teens through older adults
- Trauma Level: Minimal
