**Job Description**

- Hybrid schedule in patient homes and at home: Monday through Friday 8:30 - 5:00 pm
- Home care or hospice experience preferred

- Must be willing to travel up to one hour from home and visit patients in their homes up to 4 days/week
- Must be bilingual in Spanish
