We are working on an exciting new opportunity with a growing healthcare company that is seeking Nurse Practitioners or Physician Assistants in Seattle, WA to work visiting patients at home and virtually. There could be opportunity for quarterly travel. We would love to share more with you about this opportunity.

Job Description

• Full-time schedule in patient homes and at home: Monday through Friday 8:30 - 5:00 pm
• Home care or hospice experience preferred
• Chronic disease management experience preferred
• Must be willing to travel up to one hour from home in the Seattle area
• Must be open to travel (this could be quarterly, up to 3 days at a time and up to approximately 200 miles away)
• Must be willing to visit patients in their homes 4 days/week, 1 day remote during non-travel weeks
