    Geriatrics Nurse Practitioner

This full-time role focuses on comprehensive primary care for elderly participants, emphasizing collaboration, cultural competence, and continuity of care.

    Job Description

    • Conduct initial and quarterly examinations for new enrollees during the first year
    • Provide care for acute illnesses, episodic visits, and post-hospitalization follow-ups
    • Perform nursing home and home visits per state guidelines and participant needs
    • Participate in after-hours on-call coverage
    • Deliver patient education to participants, caregivers, and team members
    • Maintain accurate and timely documentation both onsite and offsite
    • Uphold standards outlined in the NP/MD Collaborative Practice Guide
    • Travel locally as required and comply with fit testing for respiratory protection

    Requirements

    • Master of Science in Nursing (MSN)
    • Board Certification as a Nurse Practitioner in Massachusetts
    • Active MA license and DEA prescribing authority
    • Current CPR certification
    • Credentialed or eligible for credentialing at affiliate Long Term Care facilities
    • Minimum one (1) year of experience with a geriatric population
    • Bilingual in Vietnamese or Khmer is required
