Primary Care Physician (IM/FM)

Sun River Health is looking to hire an accomplished full-time Primary Care Physician (IM/FM) to join our team at our Yonkers, Park Care site. The Primary Care Physician is responsible for managing a diverse patient panel with multiple co-morbidities in a busy Federally Qualified Health Center. Care is delivered in collaboration with advanced practice providers and on-site support staff, with an emphasis on preventative health and quality patient care.
Job Description
- Participate in planning and developing health care services, treatment of conditions and illnesses within scope.
- Diagnose and provide treatment plans for patients.
- Collaborate and supervise advanced provider clinicians.
- Complete and participate in chart review process.
- Referral and follow-up of patients when needed.
- Collaborate with other members of the Health Center in order to maintain high quality standards and achieve client satisfaction.
- Clinical supervision: Overseeing the clinical work of students or new providers, ensuring they adhere to best practices and organizational standards.
- Mentorship and guidance: Providing mentorship and guidance to students or new providers, helping them develop their clinical skills and professional behavior.
- Educational support: Facilitating educational sessions, workshops, or seminars to enhance the learning experience of the mentees.
- Performance evaluation: Assessing the performance of students or new providers, providing constructive feedback and documenting their progress.
- Patient care: Ensuring high-quality patient care is maintained while supervising and mentoring students or new providers.
- Collaboration: Working closely with academic institutions and other healthcare professionals to coordinate the educational activities and ensure a comprehensive learning experience.
Requirements
- Doctorate Degree (MD, DO)
- Active/Valid license as a MD or DO in New York State, DEA, Board certification
- Bilingual in English and Spanish (preferred)
- Experience in HIV care and MAT (Medication Assisted Treatment) (preferred)
- Demonstrated ability to work with diverse patient population in community health setting.
